Description
Running from the ischial tuberosity to the proximal leg, the semitendinosus appears as a long, slender hamstring belly on the posteromedial thigh, tapering into a cordlike distal tendon as it approaches the knee. Lateral to it, the biceps femoris forms the dominant posterolateral contour and tracks toward its insertion on the fibular head, framing the lateral aspect of the knee joint. Superiorly the pelvis and proximal femur provide the origin and bony backdrop, while inferiorly the tibia and fibula define the leg axis, with the fibula positioned laterally and the tibia more medial and weight-bearing. A clear bony chain continues into the ankle and foot, including talus and calcaneus. Understanding semitendinosus morphology matters because its long distal tendon is frequently harvested for autograft in anterior cruciate ligament reconstruction, and surgeons must anticipate its course toward the pes anserinus on the anteromedial proximal tibia while protecting the saphenous nerve branches. Sports clinicians will also recognize the proximal hamstring origin at the ischial tuberosity as a common site for tendinous avulsion and chronic tendinopathy, often presenting with deep gluteal pain and weakness in hip extension. Common injury patterns differ by compartment. Biceps femoris strains cluster laterally near the myotendinous junction, while semitendinosus and semimembranosus injuries more often localize medially.
