Description
Seen from the posterior aspect of the male lower limb, the semitendinosus is highlighted as a superficial medial hamstring arising from the ischial tuberosity and descending inferomedially toward the knee. Its fusiform belly lies medial to the long head of biceps femoris and overlies the broader, flatter semimembranosus, with all three spanning the posterior thigh from pelvis to leg. Distally, the semitendinosus tendon tracks posterior to the medial femoral condyle, crossing the knee joint line to continue toward its anteromedial tibial insertion (pes anserinus) even though the insertion is partly concealed in a strict posterior view. Bone landmarks including the pelvis, femur, tibia, fibula, and the hindfoot provide orientation for proximal and distal attachments. Clear landmarks. This posterior perspective helps you teach and recognize why semitendinosus is commonly injured and frequently harvested. Most hamstring strains occur near the proximal myotendinous junction during sprinting or kicking, and the shared ischial origin makes proximal avulsion fractures an important differential when posterior thigh pain follows a sudden eccentric load. The semitendinosus tendon is also a standard autograft for ACL reconstruction; appreciating its course relative to semimembranosus and the medial knee aids preoperative planning and postoperative education, and it explains why tenderness may localize near the posteromedial joint line.
