Description
Running along the posteromedial thigh, the semitendinosus belly sits superficial to semimembranosus and medial to the long head of biceps femoris, its fibers tapering into a long, cordlike tendon that becomes prominent as it descends toward the medial knee. Proximally, the muscle blends into the hamstring mass inferior to the gluteal fold, while distally the tendon passes posterior to the medial femoral condyle en route to the pes anserinus on the anteromedial tibia, deep to the crural fascia and close to the saphenous nerve. Laterally, the iliotibial tract provides a clean boundary between posterior and lateral compartments, with the femur acting as the central osseous reference for muscle orientation. Skin-level anatomy, clarified. Posterior surface anatomy of semitendinosus matters because that distal tendon is a common graft source for anterior cruciate ligament reconstruction, and its harvest corridor relates directly to the pes anserinus bursa and the course of the infrapatellar branch of the saphenous nerve. Palpation and strain patterns also track what you see here: sprinting injuries often involve the proximal myotendinous junction of the hamstrings, while medial knee pain can reflect pes anserine tendinopathy where semitendinosus converges with sartorius and gracilis. Clinically, the semitendinosus contour helps distinguish medial hamstring bulk from the lateral biceps femoris when assessing posterior thigh swelling or post-injury ecchymosis.
