Description
Arising from the ophthalmic artery, the short posterior ciliary arteries form multiple small trunks that approach the globe posteriorly and pierce the sclera in a tight ring around the optic nerve. Each vessel courses anterior to the optic nerve sheath but posterior to the equator of the eye, then spreads within the choroid between sclera and retina. The cornea, iris, and pupil orient the anterior segment, while the posterior pole is dominated by the sclera, choroidal vasculature, and the optic nerve exiting at the optic disc. Small-caliber branches read as arterioles as they arborize toward the choriocapillaris. Dense vascular territory. Short posterior ciliary arteries matter because they are the principal arterial supply to the choroid and the prelaminar and laminar portions of the optic nerve head. Compromise of this circulation underlies the classic teaching model for nonarteritic anterior ischemic optic neuropathy, and it also explains why giant cell arteritis can produce abrupt, profound vision loss when these branches or their parent circulation become inflamed or occluded. Their clustered scleral entry points around the nerve make them vulnerable landmarks in posterior segment surgery and a relevant consideration in posterior ciliary nerve block and other retrobulbar procedures. Ophthalmology and neuroanatomy courses use this view to teach posterior segment vascular anatomy, the circle of Zinn-Haller concept at the optic nerve head, and the layered relationship of sclera, choroid, and retina. Medical publishers can pair it with clinical discussions of ischemic optic neuropathy, choroidal infarction, or complications of posterior scleritis, while clinicians may reference it when explaining optic disc perfusion and risk factors for optic nerve head ischemia.
