Description
Arising from the ophthalmic artery, multiple short posterior ciliary arteries course anteriorly around the optic nerve and perforate the sclera at the posterior pole, clustered near the peripapillary region. From an anterior perspective, their entry points are seen posterior to the globe but oriented toward the viewer, with branches radiating to supply the choroid deep to the sclera and external to the retina. The illustration juxtaposes external ocular landmarks, including the cornea, blue iris, pupil, and sclera, with a cutaway interior that traces arterial (red) and venous (blue) pathways toward the posterior segment. Spatially, the ciliary vessels sit posterior to the iris and ciliary body, yet they feed tissues that extend anteriorly beneath the retinal pigment epithelium. Clinical relevance centers on perfusion of the choroid and optic nerve head. These arterioles contribute to the circle of Zinn Haller and are implicated in anterior ischemic optic neuropathy when posterior ciliary circulation fails, a pattern clinicians recognize in giant cell arteritis and other hypoperfusion states. Their segmental distribution also explains why choroidal infarcts can appear wedge-shaped, and why surgical maneuvers near the posterior sclera, including scleral buckling or posterior sclerotomy, demand respect for these perforators.
