Description
Running as a narrow longitudinal band along the posterior neck, the spinalis cervicis sits medial to the longissimus cervicis and deep to the overlying splenius cervicis, tracking close to the cervical spinous processes. Inferiorly it blends with the spinalis thoracis at the cervicothoracic junction, while superior fibers approach the upper cervical vertebrae beneath the nuchal ligament and adjacent semispinalis capitis. Posterior elements of the cervical vertebrae are visible through the semi-transparent rendering, with the occiput superior and the upper thorax, ribs, and scapulae inferior and lateral, framing the paraspinal compartment. Distinguishing spinalis cervicis from neighboring extensors matters because it is the medial column of the erector spinae and behaves differently under load than the more lateral longissimus and iliocostalis. In posterior cervical approaches, midline dissection follows the nuchal ligament and spinous processes, and recognizing where spinalis fibers run helps explain why subperiosteal stripping and prolonged retraction can contribute to postoperative axial neck pain.
