Description
Centered on the thoracoabdominal trunk, the superior vena cava descends in the right superior mediastinum while the inferior vena cava ascends anterior to the vertebral column, passing through the caval opening of the diaphragm to reach the right atrium. Blue systemic veins track alongside the red arterial tree, with paired common iliac veins converging in the pelvis to form the inferior vena cava, and longitudinal channels coursing adjacent to the ribs and vertebrae consistent with the azygos system and posterior intercostal veins. White bony landmarks, including the rib cage, thoracic vertebrae, lumbar vertebrae, sacrum, and iliac crests, anchor vessel position in true anatomical space. A trunk-focused caval overview matters because it clarifies the two major venous return pathways and their key relationships to the diaphragm, spine, and thoracic cage, the exact geometry surgeons and interventionalists rely on when planning central venous access or caval procedures. Inferior vena cava filters, thrombectomy, and stent placement all hinge on recognizing the iliocaval confluence and the segmental anatomy that predisposes to iliac vein compression (May-Thurner pattern) and extensive collateralization through azygos and paravertebral channels when the IVC is obstructed. Collaterals tell a story. This view makes them easier to teach.
