Description
Seen in true sagittal section, the occipital bone is highlighted along the posterior cranial base, with the squamous part (squama occipitalis) curving superiorly to form the occiput and the basilar part extending anteriorly toward the sphenoid at the clivus. Inferiorly, the margin of the foramen magnum is defined where the medulla oblongata transitions to the cervical spinal cord, and the occipital condyle region sits anterolateral to that opening at the atlanto-occipital junction. Anterior to the occipital region, the nasal cavity, orbit, maxilla, and mandible with dentition provide midface and jaw landmarks for orientation, while the cervical vertebral bodies align inferiorly beneath the cranial base. A small, isolated hyoid bone is included in the anterior neck, inferior to the mandible. Sagittal depiction of the posterior cranial fossa and foramen magnum matters when you are teaching craniocervical alignment, basilar invagination, and Chiari I malformation, where tonsillar descent relates directly to the bony confines of the occipital bone and the level of the foramen magnum. The occiput is also the fixed reference for interpreting flexion and extension at the atlanto-occipital joint, and it frames surgical corridors used in posterior fossa decompression and far-lateral approaches that work around the condylar region.
