Description
Running along the superior pharyngeal wall, the pharyngobasilar fascia appears as a dense fibrous membrane spanning from the base of the skull to the upper margin of the pharyngeal constrictors. In sagittal profile it lies deep to the pharyngeal mucosa, attaching superiorly to the pharyngeal tubercle of the occipital bone and blending laterally with the petrous temporal region, while inferiorly it merges with the buccopharyngeal fascia and the pharyngeal raphe. Anteriorly, the nasopharyngeal roof relates to the posterior nasal cavity and hard palate, with the hyoid bone sitting inferior and anterior to the pharynx as a key suspension point for suprahyoid and infrahyoid musculature. Bone landmarks of the cranial vault, maxilla, mandible and cervical vertebrae provide a stable spatial frame. Thin but tough. Clinically, this sheet matters because it provides the firm superior scaffold that the pharyngeal constrictors lack, and it defines how the nasopharynx maintains patency against negative pressure during swallowing and phonation. In transnasal and transoral approaches to the nasopharynx, understanding where the pharyngobasilar fascia anchors to the skull base helps predict the plane of dissection and the potential pathway for infection or tumor spread toward the prevertebral region and skull base. That relationship also informs why retropharyngeal abscesses can track superiorly and why skull base osteomyelitis or nasopharyngeal carcinoma may present with deep neck symptoms.
