Description
Running centrally through the posterior knee, the popliteal artery continues from the femoral artery after it passes through the adductor hiatus and descends deep in the popliteal fossa, closely accompanied by the popliteal vein. Superficially, the small saphenous vein typically drains into the popliteal vein, while the tibial nerve lies more superficial and slightly lateral in the fossa, a relationship that matters when approaching the neurovascular bundle from behind. Inferior to the joint line, the popliteal artery gives off genicular branches that arc medially and laterally around the femoral condyles and tibial plateau before continuing to its terminal bifurcation into the anterior tibial artery and tibioperoneal trunk. Posterior visualization of this vessel matters because the popliteal artery is fixed proximally at the adductor hiatus and distally at the soleal arch, making it vulnerable to traction and intimal injury with posterior knee dislocation, even when distal pulses appear present. Genicular collateral channels become a practical teaching point in chronic femoropopliteal occlusive disease, where reconstitution of distal flow may occur around the knee. Needle placement, posterior arthroscopic portals, and exposure for popliteal artery entrapment syndrome all benefit from a clear understanding of the artery’s depth relative to the popliteal vein and tibial nerve. Depth is the point.
