Profunda Brachii Vein, Posterior View

Description

Running along the posterior compartment of the arm, the profunda brachii vein (deep vein of the arm) courses adjacent to the humerus in close company with the profunda brachii artery within the radial (spiral) groove. Proximally it lies inferior to the scapula and posterior to the surgical neck region of the humerus, then tracks distally toward the posterior elbow where it communicates with the deep venous channels around the olecranon. Superficial draining pathways of the posterior forearm and hand can be appreciated overlying the radius and ulna and continuing into the dorsal venous network across the metacarpals and phalanges. Bone landmarks anchor the view, including the humeral shaft, the proximal ulna and radius at the elbow, and the carpal rows distally. Posterior visualization of the profunda brachii vein matters because it fixes the venous anatomy to the same corridor used by the radial nerve and profunda brachii artery, a relationship that explains bleeding patterns and neurologic symptoms after midshaft humeral fractures. Injury near the radial groove can compromise the deep venous outflow while also endangering the radial nerve, producing posterior arm hematoma with wrist and finger extension weakness. For surgeons and proceduralists, the deep brachial vessels also sit in the field during posterior approaches to the humerus and during exposure of the distal humerus and elbow.
