Description
Beneath the lateral skin surface of the male forearm and hand, the pronator quadratus is isolated on the distal volar forearm, spanning transversely from the anterior distal ulna to the anterior distal radius. The muscle lies immediately proximal to the carpal bones and deep to the flexor tendons that course toward the wrist and digits, while the radius sits lateral to the ulna in anatomical position and rotates around it during pronation. Proximal landmarks extend to the elbow, where the distal humerus meets the proximal radius and ulna, and the bony chain continues distally through the radiocarpal region into metacarpals and phalanges. Small muscle. Big landmark. Pronator quadratus matters because it stabilizes the distal radioulnar joint while generating pronation, and it forms a familiar muscular floor encountered during volar approaches to distal radius fixation. In a standard Henry approach for distal radius fractures, surgeons elevate pronator quadratus off the radius to apply a volar plate, and postoperative weakness or tethering can affect pronation strength and DRUJ mechanics, so appreciating the muscle’s distal ulnar origin, radial insertion, and close proximity to the interosseous border helps frame both exposure and repair.
