Description
Positioned deep to the superficial back musculature, the quadratus lumborum is presented on the posterior abdominal wall as a paired, roughly quadrangular muscle flanking the lumbar vertebral column. Each quadratus lumborum spans from the iliac crest inferiorly to the twelfth rib superiorly, with medial fascicles attaching along the transverse processes of L1 to L4, so its bulk sits lateral to the spinous processes and medial to the lateral abdominal wall. Posteriorly, the lumbar spine forms the midline bony reference, and the highlighted muscle lies just inferior to the lower thoracic region and superior to the pelvis. Fiber direction reads predominantly cranio-caudal with a slight medial inclination toward the lumbar transverse processes. Clinically, this view helps clarify why quadratus lumborum spasm can mimic flank or low back pain and why tenderness often localizes just above the iliac crest lateral to the erector spinae mass.
