Description
Arising from the right ventricle, the pulmonary trunk is shown at its root as it passes superiorly and slightly leftward from the infundibulum (conus arteriosus) toward the level where it will bifurcate into the right and left pulmonary arteries. The pulmonary valve sits at the ventriculoarterial junction, positioned anterior to the aortic root, with the ascending aorta immediately posterior and to the right in the superior mediastinum. Portions of the superior vena cava and brachiocephalic veins lie to the right of the great arterial outflow, while the rib cage and sternum frame the thoracic inlet and anterior mediastinum. Color convention distinguishes arterial and venous pathways for quick orientation. Attention to the pulmonary trunk root matters because it is the surgical and imaging gateway to right ventricular outflow tract pathology. Congenital lesions such as pulmonary valve stenosis, infundibular obstruction in tetralogy of Fallot, and post-repair pulmonary regurgitation all center on this junction, and their relationships to the aortic root guide echocardiographic windows, CT angiography interpretation, and operative planning. This is the segment cannulated and cross-clamped in many cardiopulmonary bypass strategies for right-sided or pulmonary artery procedures. Short segment. High stakes.
