Description
Arising from the lateral margin of the sacrum and coccyx, the sacrospinous ligament runs anterolaterally to its narrow insertion on the ischial spine, forming a taut, triangular band on the posterior pelvis. From this lateral male perspective, the ischium and adjacent ilium frame the ligament inferior to the sacroiliac region and posterior to the acetabulum. The femoral head and acetabular rim sit anterolateral to the ligament, placing the structure in clear relation to the hip joint line. Bony landmarks dominate. Clinically, the sacrospinous ligament is a key boundary for the greater and lesser sciatic foramina when considered with the sacrotuberous ligament, and it sits immediately adjacent to the course of the pudendal nerve as it hooks around the ischial spine between the piriformis and obturator internus. That relationship drives real-world risk: pudendal nerve entrapment and iatrogenic injury during sacrospinous ligament fixation for vaginal vault prolapse. This lateral angle makes the ischial spine easy to pick out, which is exactly the landmark used for fluoroscopic or palpation-guided pudendal nerve block and for orienting posterior pelvic surgical approaches.
