Description

Framed in anterior orientation, the large intestine is presented from the ascending colon through the transverse and descending segments to the sigmoid colon and proximal rectum, with haustra and taeniae coli defining the contour and epiploic appendages dotting the serosal surface. Across the left lower quadrant, the sigmoid arteries course within the sigmoid mesocolon toward the bowel wall, forming serial arterial arcades that run parallel to the colon before sending straight vessels (vasa recta) to the marginal artery and muscularis. Companion venous channels track alongside the arterial branches and converge toward the inferior mesenteric venous drainage. Clear relationships. Attention to the sigmoid arterial pattern matters because this is the segment most often mobilized in sigmoidectomy and low anterior resection, where division of the inferior mesenteric artery versus preservation of the left colic artery determines perfusion to the distal colon. The arcade and vasa recta arrangement also explains watershed vulnerability at the rectosigmoid junction (Sudeck’s point), a familiar site for ischemic colitis when flow is compromised by hypotension or atherosclerotic disease. For endovascular and operative planning, the anterior view helps map where ligation points fall relative to the mesenteric border and the marginal artery. Ideal for teaching hindgut vascular anatomy in gross anatomy, surgical anatomy, and colorectal fellowship curricula, and for illustrating manuscripts on ischemic colitis, diverticular disease, or anastomotic leak risk after colorectal resection.
