Description
Framed by an overlying layer of anterior lower abdominal skin and pubic hair, the os coxae (ilium, ischium, and pubis) form the lateral walls of the pelvis, meeting anteriorly at the pubic symphysis while the sacrum and coccyx sit posteriorly in the midline. The iliac crests mark the superior margin, and the acetabula occupy the inferolateral aspect where the three hip bone components converge. Medially, the pelvic inlet transitions into the lesser pelvis, bounded by the arcuate line and pecten pubis, with the obturator foramina positioned inferior to the superior pubic rami. Landmarks read cleanly. Clinical teaching often hinges on how palpable surface anatomy maps to these bony points, and the skin overlay makes that translation direct: the anterior superior iliac spine, iliac crest, and pubic tubercle are the same reference points used for pelvic examination, gait assessment, and incision planning. Obstetrics and gynecology also depend on an accurate sense of the relationship between the sacral promontory, pubic symphysis, and ischial spines when discussing pelvic dimensions and the mechanics of labor. For trauma and orthopedics, the continuity of the pelvic ring from pubis to sacrum underlies patterns of instability in pubic rami fractures and sacroiliac disruption. Orientation matters.
