Description

Running in the midline just inferior to the mandibular symphysis, the submental vein is traced across the submental triangle, hugging the inferior border of the mandible before coursing posterolaterally toward its junction with the facial vein. Superiorly, venous tributaries from the skin and soft tissues of the chin communicate with the anterior facial venous network, while inferiorly the vessel lies superficial to the suprahyoid region where it relates to the mylohyoid and the submental lymph nodes (not shown). Blue-coded venous channels of the face and neck frame the vessel, with red-coded arteries and the facial skeleton of the maxilla and mandible providing fixed landmarks for orientation. Bone first. Clinically, this anterior perspective matters because the submental vein is a consistent structure encountered during submental and submandibular approaches, including platysmaplasty, submental lipectomy, and level IA lymph node dissection for oral cavity malignancy. Injury can lead to brisk venous bleeding and troublesome postoperative hematoma in a tight cervical compartment, and its proximity to the marginal mandibular branch of the facial nerve makes it a useful landmark when planning superficial dissection planes. Venous drainage patterns here also explain how infections of the lower lip and chin can track toward the facial venous system and into deeper cervical pathways.
