Emerging in the midline of the suprahyoid neck, the submental vein courses posteriorly within the submental triangle, just inferior to the mandibular symphysis and the mentum, then turns laterally toward the superficial venous system of the face. From an inferior perspective, its relationship to the anterior belly of the digastric and the mylohyoid region can be inferred, with the vessel running superficial to the floor of the mouth and deep to the subcutaneous tissues of the submental region. Surrounding landmarks include the body of the mandible superiorly, the hyoid region inferiorly, and the cervical vertebrae posteriorly, with the facial vein and internal jugular vein represented as larger caliber drainage routes in the neck. Blue venules converge from the lower lip and chin to form the submental channel. A small vessel, but a surgical nuisance. Venous anatomy under the chin matters because it sits directly in the line of common access and aesthetic approaches, from submental liposuction and rhytidectomy adjuncts to open reduction and internal fixation of parasymphyseal mandibular fractures. Injury to the submental vein or its tributaries can produce brisk superficial bleeding and postoperative hematoma in a tight fascial compartment, and the same drainage pathway can transmit infection from the lower lip, mandibular incisors, or submental skin into the facial venous network. For head and neck educators, this inferior angle also reinforces how superficial veins relate to bony landmarks when the mandible is palpated or retracted. Use this artwork in gross anatomy teaching for the submental triangle, facial venous drainage, and surface anatomy of the anterior neck, or in surgical training materials covering submental incisions, flap elevation planes, and hemostasis around the mandibular symphysis.
