Description
Running deep to the rectus abdominis, the superior epigastric artery descends from the internal thoracic artery and enters the rectus sheath just inferior to the costal margin. From an anterior perspective in a male torso, you would expect to follow it along the posterior surface of the rectus muscle, medial to the linea semilunaris, as it gives muscular and cutaneous branches toward the anterior abdominal wall. Superiorly it lies posterior to the costal cartilages and xiphoid region, while inferiorly it approaches its anastomosis with the inferior epigastric artery in the periumbilical area. Small-caliber arterioles radiate laterally. This is the arterial pedicle that explains why the upper rectus often survives when the inferior epigastric system is interrupted, and it is a key orientation point when teaching the segmental blood supply of the anterior abdominal wall. In open and laparoscopic subxiphoid and upper midline approaches, injury to the superior epigastric vessels can drive troublesome rectus sheath hematoma, and their position within the sheath helps predict where bleeding will track. The illustration also supports discussions of collateral flow between the internal thoracic and external iliac systems, a concept that becomes concrete when you can see the superior and inferior epigastric connection.
