Description
Arising from the femoral artery just inferior to the inguinal ligament, the superficial epigastric artery courses anteriorly and superiorly within the superficial fascia of the lower anterior abdominal wall. Its trunk runs medial to the anterior superior iliac spine and crosses toward the midline, sending small cutaneous branches that anastomose with superficial and deep epigastric territories. In the same anterior field, the abdominal aorta and its iliac bifurcation sit deep to the abdominal wall, while the inferior vena cava and iliac veins provide the major venous return pathways seen in parallel. Bony landmarks such as the sternum, costal cartilages, ribs, and pelvis anchor orientation. Surgeons and proceduralists care about this vessel because it defines a bleeding risk in the groin and suprapubic region where superficial dissection is common. During inguinal hernia repair, femoral access, or lower abdominal flap elevation, injury to the superficial epigastric artery can cause brisk subcutaneous hemorrhage and troublesome hematoma, and its variable origin from the femoral artery (often near the saphenous opening) makes surface anatomy unreliable. Its anastomoses also explain why superficial abdominal wall perfusion can persist when one channel is compromised. Small vessel, real consequence.
