Arising on the anteromedial aspect of the knee, the superior medial genicular vein courses proximal to the medial femoral epicondyle and deep to the distal vastus medialis, collecting venules from the superomedial capsular and periarticular tissues. It runs in close company with the superior medial genicular artery, then tracks posteriorly toward the popliteal venous system while maintaining anastomoses within the genicular venous network. The patella, distal femur, and proximal tibia frame the joint anteriorly, with the medial retinaculum and adjacent tendinous fibers marking the superficial plane. Small. Variable. Connected.
