Description
Suspensory ligaments (Cooper ligaments) are rendered as dense fibrous septa traversing the subcutaneous tissue from the dermis of the anterior breast to the deep fascia overlying pectoralis major. Between these fibrous bands sit the lobules of glandular tissue and their lactiferous ducts, arranged radially toward the nipple and areola on the anterior surface. Red arterial and venous channels course through the stroma, while green lymphatic vessels and collectors track toward the axillary tail superolaterally and toward parasternal pathways medially. Deep to the gland, the retromammary space separates breast parenchyma from the pectoral fascia. Cooper ligaments matter because they are the mechanical link between skin and parenchyma, so distortion of these septa is one of the anatomic reasons you see skin tethering and dimpling with invasive breast carcinoma or post-inflammatory scarring. A short, unforgiving band. The same anatomy also explains why mastectomy and breast-conserving surgery must respect the plane of the pectoral fascia and retromammary space, and why edema or lymphatic obstruction can present as peau d’orange when dermal lymphatics are involved.
