Description
Arising variably from the brachiocephalic trunk, aortic arch, or less often the right common carotid artery, the thyroid ima artery ascends in the midline on the anterior surface of the trachea toward the inferior poles and isthmus of the thyroid gland. Superiorly, the hyoid bone and mandible frame the upper neck, while the common carotid arteries course bilaterally and the internal jugular veins lie lateral to them within the carotid sheath. Fine cervical nerves track in the cervical fascia planes, with the trachea and laryngeal framework occupying the central axis. Midline anatomy. Unforgiving. For anterior neck procedures, this variant vessel changes the risk profile. A thyroid ima artery can be a source of brisk hemorrhage during tracheostomy or cricothyrotomy because it may cross the 2nd to 4th tracheal rings or run directly over the pretracheal fascia where the incision and dilatation occur. It also matters in thyroidectomy and central neck dissection, where a midline inferior arterial pedicle can be mistaken for a small inferior thyroid branch, then retract and bleed into the superior mediastinum. Use this plate in surgical anatomy teaching for ENT and general surgery trainees, in anesthesia and critical care modules on airway access, and in atlas or textbook sections on thyroid vascular variants and their relationship to the trachea and carotid sheath. It also supports radiology correlation when discussing contrast-enhanced CT angiography of lower cervical vessels and unexpected midline arterial enhancement anterior to the trachea.
