Description
Arising from the inferior portion of the pancreatic head, the uncinate process (processus uncinatus) curves posteriorly and medially, forming the characteristic hooked projection seen best when the gland is oriented in anatomical position from the anterior aspect. Superior to it lies the main bulk of the head, continuous laterally with the neck of the pancreas, while the uncinate process occupies the inferomedial contour where the gland approaches the midline. Although an anterior view limits direct visualization of retroperitoneal vessels, the uncinate’s position implies its close relationship to the superior mesenteric vessels as they pass posterior to the pancreatic neck and along the medial border of the head. A small but decisive landmark. Surgeons care about this projection because it is the pancreatic tissue most intimately associated with the superior mesenteric artery and vein, and complete uncinate dissection often determines whether a pancreaticoduodenectomy achieves an R0 margin. Tumors arising in the head frequently extend into the uncinate, and on cross-sectional imaging the hook-like contour can help localize disease to the inferior head versus the neck, where involvement may threaten the portal confluence. The view also supports teaching the “mesopancreas” concept, where fibrofatty tissue posterior to the uncinate can harbor perineural spread.
