Description
Circumferential layers of the small intestinal wall are arranged around a central lumen, starting with the mucosa, including simple columnar epithelium and lamina propria, then the muscularis mucosae, submucosa, muscularis externa (inner circular and outer longitudinal smooth muscle), and an outer serosa. Branching arterial vessels course within the mesenteric connective tissue and enter the bowel wall along the mesenteric border, giving rise to submucosal arteries and smaller arterioles that project toward the mucosal plexus. Lymphatic channels and autonomic nerve fibers accompany these vessels, running parallel to the arterial branches before distributing circumferentially. Arterial anatomy is not window dressing in the small intestine. Segmental branches from the superior mesenteric artery form arcades and vasa recta, and compromise at any point along that route, embolus, thrombosis, or low flow states, can translate into mucosal ischemia long before full thickness infarction develops. Surgeons rely on this layered and vascular organization when selecting bowel for anastomosis, judging perfusion at the cut edge, and understanding why a mesenteric-sided injury can threaten a longer segment than a similar serosal insult on the antimesenteric border.
