Description
Radiographic contrast outlines the C-shaped duodenum as it leaves the pylorus and wraps around the head of the pancreas, beginning with the superior (first) part, the duodenal bulb, positioned anterolateral to the gastroduodenal artery and superior to the transverse colon. The animation tracks sequential filling of the descending (second) part along the right side of the vertebral column, then the horizontal (third) part crossing from right to left anterior to the aorta and inferior vena cava, and finally the ascending (fourth) part rising to the duodenojejunal flexure, supported by the suspensory muscle of the duodenum (ligament of Treitz). As contrast progresses, the mucosal pattern and luminal caliber change with peristaltic waves, clarifying the transition from the smooth bulb to the more feathery folds of the distal segments. Clear landmarks appear. Fast. For upper GI fluoroscopy teaching, the duodenal bulb matters because it is the common site of peptic ulcer disease, where a crater, deformity, or persistent niche alters the normal rounded contour and can tether the pyloric channel. Watching the bolus advance also helps you appreciate fixed narrowing versus transient spasm, and it supports pattern recognition for postbulbar strictures, duodenitis, and obstruction at the third part where the superior mesenteric artery can compress the duodenum (SMA syndrome) against the aorta. The sequence makes malrotation and abnormal positioning of the duodenojejunal junction easier to call than in a single frame, because the expected left-sided ascent and final flexure are shown as an endpoint rather than an assumption. Ideal placements include radiology and anatomy curricula covering small bowel contrast studies, GI modules on peptic ulcer complications, and publisher graphics accompanying sections on duodenal anatomy, fluoroscopic technique, or differential diagnosis of proximal small bowel obstruction.
