Description
Anterior abdominal viscera fade to an X-ray, radiographic look as the duodenum comes forward, its superior part (duodenal bulb) leading into the C-shaped curve that wraps around the head of the pancreas. The descending (second) portion tracks inferiorly on the right side of the vertebral column, then turns medially as the horizontal (third) portion crosses leftward, with the ascending (fourth) portion rising to the duodenojejunal flexure. Spatial relationships read clearly: the pancreatic head sits medial within the duodenal sweep, while the third part lies anterior to the abdominal aorta and inferior vena cava and typically passes posterior to the superior mesenteric vessels. Motion clarifies the turns. Clinically, this is the anatomy behind many real radiology reads. Duodenal bulb ulcers classically erode posteriorly toward the gastroduodenal artery, while the ampulla of Vater on the posteromedial wall of the second part explains why gallstone obstruction and pancreatitis so often converge at the same junction. The sequence makes the retroperitoneal fixation of the second and third parts easier to grasp than a static plate, and it helps explain compression patterns such as superior mesenteric artery syndrome, where the third part is trapped between the SMA anteriorly and the aorta posteriorly.
