Description
Rendered in an X-ray, radiographic style, the duodenum appears as a C-shaped loop immediately distal to the pylorus, curving rightward and posteriorly around the head of the pancreas before turning left to meet the proximal jejunum at the duodenojejunal flexure (ligament of Treitz). The sequence tracks the duodenal bulb (superior part) in the epigastrium, then follows the descending (second) portion along the right of the vertebral column, the horizontal (third) portion crossing the midline at about L3, and the ascending (fourth) portion rising to the left of the aorta. Relationships become legible as the animation advances, with the third part shown posterior to the superior mesenteric artery and vein and anterior to the aorta and inferior vena cava. Clean landmarks. Clinical reading of this region often hinges on knowing exactly where each segment lies. Ulcer disease concentrates in the duodenal bulb; the radiographic orientation helps learners anticipate where a posterior ulcer can erode into the gastroduodenal artery and where edema can narrow the lumen near the pylorus. The animated progression also clarifies why superior mesenteric artery syndrome compresses the third portion between the SMA anteriorly and the aorta posteriorly, and why annular pancreas or malrotation changes the expected sweep of the C-loop on an upper GI series.
