Description
Deep within the male pelvic basin, the levator ani complex comes into view as a muscular diaphragm spanning from the posterior surface of the pubic body and the arcus tendineus levator ani along obturator internus fascia to the coccyx and anococcygeal raphe. Fiber direction is traced in sequence through puborectalis, which forms a U shaped sling around the anorectal junction, then posteriorly into pubococcygeus and iliococcygeus as they sweep superomedial to meet their midline attachments. Posterolateral to the levator plate, coccygeus (ischiococcygeus) fills the gap between the ischial spine and lower sacrum and coccyx, lying deep to the sacrospinous ligament.
