Description

Paired kidneys sit retroperitoneally on the posterior abdominal wall, the right typically slightly inferior to the left, with renal hila facing anteromedially toward the renal vessels and renal pelvis. As the sequence runs, urine is traced from the minor and major calyces through the funnel-shaped renal pelvis into each ureter, which descends anterior to psoas major, crosses the pelvic brim near the common iliac bifurcation, and courses inferomedially to the urinary bladder. Within the pelvis, the ureters enter the bladder wall obliquely at the posterolateral angles of the trigone, while the short female urethra continues inferiorly to the external urethral orifice in the anterior perineum. Urine flow is the focus. Clinically, following the continuous lumen from calyx to urethra helps clarify where obstruction and reflux arise: a calculus lodged at the ureteropelvic junction, at the iliac vessel crossing, or at the ureterovesical junction produces upstream hydroureter and hydronephrosis with characteristic pain patterns.
