Centered in the middle mediastinum, the heart appears in situ behind the sternum and anterior to the esophagus, with the right atrium and right ventricle positioned more anteriorly and the left ventricle forming much of the left lateral and inferior border. From the superior aspect, the ascending aorta curves into the aortic arch and continues as the descending thoracic aorta before tracking inferiorly as the abdominal aorta, while the pulmonary trunk exits anteriorly and bifurcates toward the lungs. As the sequence expands outward from the thorax into the limbs, major arteries and veins extend proximally to distally, including the subclavian and axillary vessels to the brachial circulation, and the iliac and femoral vessels into the popliteal and tibial distributions. Circulation is traced end to end. Clinical relevance is easiest to appreciate when the vascular tree is kept in correct topography relative to the thorax and extremities, because many interventions and pathologies are location-dependent: coronary artery disease concentrates on the epicardial vessels, aortic aneurysm and dissection track along the ascending aorta, arch, or abdominal segment, and venous thromboembolism often originates in the deep veins of the calf and propagates proximally.
