Description

Beginning at the common iliac bifurcation, the animation tracks the internal iliac artery as it descends posteromedial to the pelvic brim and issues parietal and visceral branches toward the rectum, pelvic floor, and perineum. Branch pathways resolve sequentially into the middle rectal artery coursing medially toward the ampulla, the internal pudendal artery passing anteroinferiorly to supply the inferior rectal artery, and the superior rectal artery continuing distally from the inferior mesenteric artery along the posterior rectal wall. Venous channels accompany the arteries and then separate into the superior rectal vein draining cranially toward the inferior mesenteric vein and portal system, and the middle and inferior rectal veins draining laterally into the internal iliac system. Spatial relationships to the bladder base, prostate, seminal vesicles, and levator ani are kept in clear anterior, posterior, and inferolateral context as vessels approach the rectal fascia and anal canal. Portosystemic communication at the rectum is the clinical anchor, because the superior rectal (portal) and middle/inferior rectal (systemic) venous pathways form a classic site for variceal dilation in portal hypertension and for clinically confusing hemorrhoidal bleeding.
