Radiographic framing centers the thymus in the anterior superior mediastinum, immediately posterior to the manubrium and body of the sternum and anterior to the pericardium overlying the great vessels. As the sequence progresses, the right and left thymic lobes are outlined as a bilobed soft-tissue structure that can project toward the cardiomediastinal contours, with subtle asymmetry between sides. Superiorly, the gland approaches the thoracic inlet near the trachea, while inferior extensions taper toward the level of the heart and upper pericardial reflection. Motion is gentle and didactic, using changing contrast and emphasis to separate thymic tissue from adjacent lung fields. Adult thymic anatomy on plain radiography is a frequent source of confusion because physiologic involution and fatty replacement reduce its conspicuity, while hyperplasia or neoplasm can restore a prominent anterior mediastinal silhouette. Small details matter. A thymoma or thymic carcinoma may present as a well-defined mediastinal mass and is classically linked to myasthenia gravis, so being able to localize the thymus relative to the sternum, heart border, and proximal great vessels helps interpret an abnormal chest X-ray and triage to CT or MRI.
