Description
Running immediately lateral to the thoracic spinous processes, the rotatores appear as short, obliquely oriented slips spanning from a transverse process to the lamina and spinous process of the vertebra above, with fibers coursing superomedially and crossing toward the midline. Superficial layers are opened to expose the deep transversospinalis group, so the rotatores sit deep to semispinalis and multifidus and medial to the rib angles where the ribs project laterally from their costotransverse articulations. Thoracic vertebrae form the bony scaffold, with spinous processes aligned posteriorly and ribs radiating laterally and inferiorly from the vertebral column. Trapezius and adjacent scapulothoracic musculature frame the region superficially around the upper back and shoulder girdle. Because the rotatores are small and segmental, this close view is often the clearest way to teach how deep paraspinal muscles generate coupled rotation and provide proprioceptive input rather than gross extension strength.
