Beneath a partially transparent male facial integument, the deep layer of the masseter occupies the lateral surface of the mandibular ramus and angle, its fibers running inferiorly from the zygomatic arch and adjoining zygomatic process toward the lateral ramus. Superiorly, the deep masseter blends with the zygomaticomandibularis (often grouped within the quadrate muscle complex in some anatomical texts), creating a muscular sling between the zygoma and mandible. Posterior to the masseteric bulk, the external acoustic meatus and deeper ear structures sit within the temporal bone, while the dentition and maxilla lie anterior and medial to the ramus. Cervical vertebrae and a portion of the clavicle and upper ribs extend inferiorly, providing bony context for head and neck alignment. Deep masseter anatomy matters when you need to localize lateral mandibular pain or plan an approach around the parotidomasseteric region. Hypertrophy of the deep masseter can mimic parotid enlargement on physical exam, and chemodenervation planning benefits from understanding the muscle’s deep compartment against the ramus to reduce the risk of diffusion toward the parotid gland and adjacent facial nerve branches.
