Running along the medial leg, the great saphenous vein ascends anterior to the medial malleolus, tracks up the medial border of the tibia, and continues over the medial knee toward the femoral region where it approaches the saphenofemoral junction. Posteriorly, the small saphenous vein courses behind the lateral malleolus and up the midline of the calf toward the popliteal fossa. Deep companion veins (venae comitantes) align with the tibia and fibula and converge proximally into the popliteal and femoral venous pathways, while the dorsal venous network of the foot feeds both superficial trunks through perforating connections. Bones of the lower limb, femur, patella, tibia, fibula, and the tarsals, metatarsals, and phalanges provide fixed landmarks for these vascular trajectories. Venous anatomy of the leg matters most when superficial and deep systems stop communicating normally. Valve failure in the great saphenous vein and its tributaries drives primary varicose veins and chronic venous insufficiency, and the course over the medial tibia explains why reflux-related dilatations become so visible in that corridor.
