Description
Arising from the descending thoracic aorta, the bronchial arteries track medially toward the tracheobronchial tree, then run along the posterior aspects of the main bronchi and proximal lobar bronchi as fine nutrient vessels distinct from the larger pulmonary arteries. Superiorly, the trachea bifurcates at the carina into right and left principal bronchi, which sit anterior to the esophagus and are flanked by pulmonary arteries and veins at the lung hila. Red systemic branches course alongside the bronchi while blue pulmonary arteries deliver deoxygenated blood to the lungs and red pulmonary veins return oxygenated blood to the left atrium. Small-caliber arterioles and peribronchial plexuses are emphasized against the broader cardiopulmonary circulation. Bronchial artery anatomy matters because it explains why hemoptysis is most often systemic in origin, with hypertrophied bronchial arteries acting as the dominant bleeding source in bronchiectasis, tuberculosis, and chronic inflammatory airway disease. Interventional radiologists rely on these pathways during bronchial artery embolization, where awareness of common variants and anastomoses with pulmonary vessels helps reduce non-target embolization and neurologic complications from inadvertent spinal cord supply via intercostobronchial trunks.
