Description
Centered in the thorax, the heart sits medial to the bilateral lungs, with the trachea descending superiorly and bifurcating at the carina into the right and left main bronchi that course inferolaterally into the hila. Slender bronchial arteries arise from the thoracic aorta (and sometimes a posterior intercostal artery), then run along the posterior aspect of the bronchi and segmental branches, forming a peri-bronchial arterial plexus that tracks distally toward the bronchioles. Pulmonary arteries accompany the bronchial tree from the right ventricle to the alveolar capillary bed, while pulmonary veins return oxygenated blood to the left atrium; the systemic bronchial vessels remain smaller-caliber but conspicuous as red channels around the airways. Bronchial artery anatomy matters because it represents the systemic blood supply to the conducting airways, visceral pleura, and supporting structures of the lung, and it is the dominant source of clinically relevant hemoptysis. In chronic inflammation (bronchiectasis, cystic fibrosis, tuberculosis) or post-thromboembolic states, hypertrophied bronchial arteries and bronchopulmonary anastomoses can drive high-pressure bleeding, guiding bronchial artery embolization planning and explaining why pulmonary arterial occlusion does not necessarily protect against airway hemorrhage. Small vessel, big consequence.
