Description

Color-coded arteries and veins course through the distal lower leg into the foot, with the anterior tibial artery continuing onto the dorsum as the dorsalis pedis artery while the posterior tibial artery passes posterior to the medial malleolus toward the plantar compartment. Distally, the dorsalis pedis gives off the arcuate artery across the tarsus and proximal metatarsals, feeding the dorsal metatarsal arteries and then dorsal digital branches to the phalanges. On the sole, the medial and lateral plantar arteries contribute to the deep plantar arch, which lies deep to the plantar aponeurosis and intrinsic muscles and sends plantar metatarsal and plantar digital arteries toward the digits. Superficial and deep venous channels accompany these pathways in parallel. Small vessels matter here. Palpating the dorsalis pedis pulse just lateral to the extensor hallucis longus tendon remains a standard bedside assessment for peripheral arterial disease and acute limb ischemia, and this view makes the surface relationship clear against the bony tarsus and metatarsals. The deep plantar arch and its perforating connections help explain why forefoot perfusion can be preserved despite proximal stenosis, and why diabetics with medial arterial calcification may show misleading ankle pressures while digital ischemia progresses. For surgical planning, the arterial layout also maps the risk zones for dorsomedial incisions, Lisfranc approaches, and flap design around the first web space.
