Description
Running along the anterolateral, superficial upper limb, the cephalic vein is traced from the dorsolateral hand and radial forearm proximally across the lateral border of the biceps brachii toward the deltopectoral groove. Near the shoulder it lies lateral to the pectoralis major and anterior to the deltoid, where it typically pierces the clavipectoral fascia to drain into the axillary vein. The basilic vein is implied on the medial side, with accessory cephalic tributaries and superficial venous connections coursing across the cubital fossa. Muscles of the arm and shoulder and accompanying yellow peripheral nerves provide landmarks for depth and orientation. Cephalic venous anatomy matters because it is a common access route for peripheral cannulation and for placement of cardiac device leads via cephalic cutdown at the deltopectoral interval, a technique that reduces pneumothorax risk compared with subclavian puncture. Variability is the point: the cephalic vein can be small, duplicated, or drain high into the axillary or even subclavian system, and the accessory cephalic vein may dominate forearm drainage. Short segment, frequent problem. Phlebitis from repeated cannulation and failure of cephalic arch outflow in dialysis access both hinge on appreciating its superficial course and proximal angulations.
