Description
Running from the dorsal venous arch of the foot proximally toward the groin, the superficial great saphenous vein ascends anterior to the medial malleolus and along the medial tibial border, while the small saphenous vein tracks posterior to the lateral malleolus and up the posterior midline of the calf. Deep to the crural fascia, paired venae comitantes accompany the anterior tibial, posterior tibial, and fibular (peroneal) arteries, converging into the popliteal vein posterior to the knee joint and continuing as the femoral vein in the thigh. Bony landmarks are kept in register, including femur, patella, tibia (medial), fibula (lateral), and the tarsals, metatarsals, and phalanges, which helps anchor the venous pathways in true anatomical position. Valves matter. Teaching venous return in the male lower limb becomes easier when you can relate perforating veins and the saphenofemoral and saphenopopliteal junctions to stable osseous reference points, because reflux patterns and varicosities follow anatomy rather than dermatome maps. This view also supports procedural planning: great saphenous vein ablation, stripping, or harvest for bypass requires an appreciation of its long subcutaneous course and its relationship to the medial knee and distal leg where iatrogenic injury to the saphenous nerve is a known pitfall. Deep venous thrombosis is not an abstract diagnosis here, the paired calf veins, popliteal vein, and femoral vein form the main conduit where thrombus propagation and embolic risk are assessed.
