Description
Layered over the calvaria, the occipitofrontalis (epicranius) spans from the frontal belly on the forehead to the occipital belly near the superior nuchal line, with the galea aponeurotica forming the tendinous sheet between them. Anteriorly, the frontalis lies superficial to the frontal bone and superior to the orbits, while posteriorly the occipitalis sits over the occipital squama just superior to the upper cervical region. Lateral margins blend toward the temporal fascia over the temporal bone, framing the parietal region. Bony landmarks such as the zygomatic bone, maxilla, and mandible orient the face, with the teeth and nasal framework providing additional midline reference. Brow elevation and scalp retraction depend on this muscle-aponeurosis unit, so the spatial continuity between frontalis and occipitalis matters when teaching expression mechanics and scalp mobility. Surgical planning does too, because the temporal branches of the facial nerve course superficially as they traverse the temporal region toward the frontalis, a common at-risk zone in coronal incisions and brow-lift approaches. Scalp lacerations also behave differently above the galea, where gaping and bleeding can be pronounced, versus deeper planes where the loose areolar tissue permits spread. Small muscle, big consequences.
