Description
Arising from the posterior tubercle of the atlas (C1), the rectus capitis posterior minor ascends superiorly and slightly laterally to attach on the medial part of the inferior nuchal line and adjacent occipital squama. The paired muscles sit deep in the suboccipital region, immediately posterior to the atlanto-occipital joints and just medial to the rectus capitis posterior major, which takes a more lateral course toward the inferior nuchal line. Surrounding osseous landmarks include the occipital bone at the skull base, the posterior arch of C1, and the upper cervical vertebrae inferiorly, with the vertebral foramen centered in the posterior elements. Small and compact, rectus capitis posterior minor is a teaching favorite because it cleanly illustrates the difference between the atlanto-occipital and atlanto-axial complexes: its line of pull supports head extension and fine postural control rather than large-amplitude rotation. Clinically, its deep location makes it relevant in posterior cervical and suboccipital pain patterns, and it is often discussed alongside the myodural bridge, a fibrous connection described between suboccipital muscles and the cervical dura that can be referenced in headache and cervicogenic symptom discussions. This is also the muscle you need to distinguish from rectus capitis posterior major during layered dissection of the suboccipital triangle boundaries and adjacent vertebral artery course.
