Description
Positioned deep in the suboccipital region, the rectus capitis posterior minor spans from the posterior tubercle of the atlas (C1) to the medial part of the inferior nuchal line on the occipital bone. From a posterior perspective, it sits medial to rectus capitis posterior major and deep to semispinalis capitis, with the paired muscles flanking the midline just inferior to the external occipital protuberance. The occiput, atlas, and the superior cervical vertebrae provide the bony context, while the upper thoracic spine and scapular outlines recede inferiorly and laterally. Small muscle, big consequences. Suboccipital anatomy matters because millimeters count near the craniovertebral junction, where the vertebral artery courses in the suboccipital triangle and the C1 dorsal ramus (suboccipital nerve) supplies the short extensors. Rectus capitis posterior minor is also clinically discussed in relation to cervicogenic headache and occipital pain patterns, and its reported connective tissue continuity with the spinal dura (the so-called myodural bridge) makes this muscle a frequent teaching point in upper cervical biomechanics. Clear depiction of its origin on C1 helps distinguish it from the posterior major, which arises from the C2 spinous process.
