Posterior Perspective of the Posterior Major Rectus Capitis

Description

Arising from the spinous process of the axis (C2), the rectus capitis posterior major courses superolaterally to insert on the lateral part of the inferior nuchal line of the occipital bone. From this posterior perspective, the paired muscles sit deep in the suboccipital region, immediately inferior to the occiput and medial to the more lateral obliquus capitis superior, with the posterior arch of the atlas (C1) and upper cervical spinous processes providing bony orientation. Inferiorly, the cervical vertebral column continues into the upper thoracic region, with the superior angles of the scapulae only partly entering the lower field. Small muscles. Big landmarks. Clinically, this is the corridor where you teach and navigate the suboccipital triangle, bounded by rectus capitis posterior major and the obliquus capitis muscles, with the vertebral artery and suboccipital nerve (dorsal ramus of C1) classically traversing the space. Hypertone or trigger points in rectus capitis posterior major are a common generator of cervicogenic headache and occipital pain patterns, and the muscle’s attachment along the nuchal line helps explain symptom referral with sustained upper cervical extension. The relationship to C1 and C2 also matters in posterior approaches to the craniovertebral junction, where muscle-sparing dissection and clear identification of the inferior nuchal line reduce orientation errors.
