Description
Cut through in a longitudinal section, the male kidney reveals the renal cortex superficially and the medulla deep to it, with medullary pyramids converging toward the renal papillae and minor calyces. At the medial border, the renal hilum is opened to show the renal vein as a wide-caliber blue channel coursing anterior to the renal artery before exiting toward the inferior vena cava. Segmental arterial branches in red fan into interlobar arteries between pyramids, then arch as arcuate arteries at the corticomedullary junction, while venous tributaries mirror this path back toward the hilum. The ureter begins inferiorly from the renal pelvis. Emphasis on the renal vein in this sectional perspective helps clarify the classic hilar arrangement used in surgery and cross-sectional imaging: vein anterior, artery posterior, pelvis most posterior. That ordering matters during donor nephrectomy, partial nephrectomy, and hilar dissection where early venous control can limit blood loss and where anatomic variants (multiple renal veins, circumaortic left renal vein, retroaortic left renal vein) change the operative plan. It also anchors interpretation of venography or contrast-enhanced CT when evaluating renal vein thrombosis, nutcracker syndrome, or tumor thrombus extension from renal cell carcinoma.
