Description
Arising along the deep surface of the posterior axillary fold, the thoracodorsal vein tracks with the thoracodorsal artery on the lateral thoracic wall, then courses superiorly and anteriorly toward the axillary vein. From a posterior, superior-oblique back view in a male model, the vessel runs medial to the proximal humerus and deep to the scapula’s lateral border, in the corridor between the latissimus dorsi and teres major. Surrounding landmarks include the scapulae over the posterior ribs, the vertebral column in the midline, and the lower cervical region where more proximal venous trunks converge. Small tributaries from the latissimus dorsi and adjacent posterior shoulder musculature feed into the main channel. Clear anatomy. Surgical planning often hinges on this anatomy because the thoracodorsal vein is the principal venous outflow of the latissimus dorsi flap, traveling with the thoracodorsal nerve and artery as a compact pedicle that is dissected during breast reconstruction and posterior trunk coverage. In axillary lymph node dissection and sentinel node procedures, recognizing the thoracodorsal bundle helps prevent iatrogenic injury that can compromise flap options and contribute to postoperative bleeding or seroma. This posterior perspective also clarifies the relationship between the thoracodorsal vessels and the axillary vein at the apex of the axilla, a frequent site for vascular control during trauma exposure. Ideal for gross anatomy lab teaching on the posterior thorax and axilla, and for surgical atlases covering latissimus dorsi flap elevation, axillary node clearance, and posterior shoulder approaches. It also supports radiology education when correlating axillary vascular anatomy on contrast-enhanced CT or MR angiography by anchoring the thoracodorsal pathway to fixed bony landmarks like the scapula and ribs.
