Description

Running obliquely along the deep surface of latissimus dorsi, the thoracodorsal vein ascends from the posterolateral trunk toward the axilla, traveling with the thoracodorsal artery and thoracodorsal nerve in a tight neurovascular bundle. From this posterior perspective, the vessel is seen coursing superiorly and slightly anteriorly around the inferior angle of the scapula, then turning medially to join the axillary vein near the lateral border of the first rib and pectoralis minor region. Bony landmarks frame the pathway, including the thoracic spinous processes in the midline, ribs laterally, the scapula overlying the posterior thoracic wall, and the proximal humerus lateral to the axillary space. Small venous tributaries from the latissimus dorsi and adjacent posterior shoulder region converge into the main channel. For surgeons working in the axilla and posterior shoulder, this anatomy is not academic. The thoracodorsal vein is a key landmark during axillary lymph node dissection and sentinel node biopsy, where preserving the thoracodorsal neurovascular pedicle reduces the risk of denervating latissimus dorsi. It also defines the dominant venous outflow of the latissimus dorsi flap, so injury or thrombosis can compromise pedicled breast reconstruction or coverage of posterior trunk defects. Exposure is often gained by following the axillary vein and identifying the thoracodorsal vessels as they enter the posterior axillary fold. A clean orientation point.
