Description
Shown from the knees to the feet in anatomical position, the male lower legs are rendered with semi-transparent skin to expose the superficial and deep venous pathways. Medially, the great saphenous vein tracks anterior to the medial malleolus and ascends along the tibial border toward the knee, while laterally the small saphenous vein courses posterior to the lateral malleolus and rises in the midline of the calf toward the popliteal fossa. Between these superficial trunks, communicating perforator veins traverse the deep fascia to link with paired deep veins accompanying the anterior tibial, posterior tibial, and fibular (peroneal) arteries, with venous valves indicated along the channels to reinforce proximal flow. Clinical teaching benefits from a distal-to-proximal overview like this because it clarifies where reflux and obstruction present on exam and ultrasound mapping. In chronic venous insufficiency, valve failure commonly affects the saphenofemoral or saphenopopliteal junctions and their tributaries, producing varicosities that follow the long axis of the great or small saphenous systems and edema that worsens in dependent position. Perforator incompetence in the gaiter area of the medial distal leg helps explain venous stasis dermatitis and ulceration near the medial malleolus. Form follows function.
